The Geriatric Dementia Program developed by the Timmins Family Health Team works with local community services to provide clients with care coordination, caregiver support and most importantly client centered care. This program will allow clients to remain as independent and healthy as possible, with emphasis on providing ongoing support for the caregivers.
The Program consists of a multidisciplinary team which receives referrals from our family health team physicians and nurse practitioners. Our team consists of 1 physician, 1 geriatric nurse and 1 social worker who communicate directly with the patient’s primary health care provider. Our program provides a full time geriatric nurse to the programs patients and caregivers. We provide a comprehensive plan of care and education through the journey of memory loss.
Our goal is to assist elderly patients to stay at home as long as they can in a safe environment with the assistance of their family/friends, our team and community partners. We assist patients and their caregivers so that we can prevent crisis situations at home as well as admissions to hospital. We have worked with our community partner The Alzheimer’s Society to provide education for patients and caregivers on living life with dementia.
