Becoming a Patient
To be eligible to become a patient of The Timmins Family Health Team you must register with the Ontario Health Care Connect program.
All Physicians and Nurse Practitioners will be using the Ontario Health Care Connect as their primary source for rostering new patients.
Health Care Connect is a provincial program which helps Ontarians who are without a family physician or nurse practitioner to find one. People without a primary health care provider are referred to a family physician or nurse practitioner in their community who is currently accepting new patients.

Answer: The Family Health Teams are part of the Ministry of Health's transformation of primary health care. There are 150 teams in Ontario. The Timmins Academic Family Health Team is comprised of a group of family physicians in the Timmins area. Also operating out of these offices are mental health counsellors, dietitians, nurse practitioners and pharmacists. There are no charges for this service if you are eligible for OHIP.
Answer: Some services are by referral only and you do have to be a patient of one of the local family physicians associated with the FHT. There are, however, some groups and programs that are open to the community. Please check our website under 'Events' for details.
